- Child Health Examination Form (This form must be completed by your child's physician)
- SMART Tuition Registration Form
Checklist for Admission: - Completed Application (which includes all of the documents above)
- Birth Certificate
- Social Security Card
- Baptismal Certificate (if child is Catholic and baptized)
- Immunization Record
- Most recent Report Card, (or most recent academic record if entering Kindergarten)
- Registration Fee of $325 (Money Order or Check preferred)
An extrance exam is given for students entering 2nd grade or higher.
